Hold Harmless Agreement Reminder
*ATTENTION: You must have "Hold Harmless Agreement" signed and on file with ICPHA prior to showing or working at any ICPHA function. If you are a minor (under age 18) your parent or legal guardian must be present at your first show to personally sign the form. NO EXCEPTIONS! If this is not done, you will not be able to show or work. |
